Vision screening has just become simpler and quicker. Two of the stations, +Lens and Near Vision have been eliminated and there are now only two stations for vision. This has been approved by our consulting optometrist, Lion Dr. Roger Ede. Plain English: only do Distance Vision and Stereo.
This means the cards have changed, the data entry form has changed, and procedures have changed to reflect this.
We are stressing the screening of the grades decided upon by the State DOH – PreK(4 yrs), K, 1, 3, 5, 7, and those that were not screened the year before. There really is no reason past the 1st grade to test every year, all you are doing is expending time and resources.
We are also stressing the inclusion of the teacher ABC survey for those students whom the teachers feel have eye problems that may be affecting the student’s ability to learn. Coordinators PLEASE include this in the presentations you make to your school personnel. Although it is clearly written on the form, it is NOT to be filled in for every student and for every teacher.
Data entry people enter the three digit alpha numeric code(s) in the comment section of the form and have the school retain the ABC that will be attached to the student referral.
For all who have old vision and hearing forms, including permission slips, please delete/throw them away. The new forms for everything are included in this folder.
